LEGAL DISCLOSURE Application is made to N.V.G.E.B.E. Gemeenschappelijk Elektriciteitsbedrijf Bovenwindse Eilanden (NV GEBE) for the supply of electric and water service. Upon acceptance of this application, by NV GEBE, the applicant agrees to pay for electric and water service provided subject to the Rates Schedule(s) effective during the period of service. NV GEBE’s General Terms and Conditions provide that a security deposit is required of a residential customer under certain conditions. It is the responsibility of the customer to: (1) provide NV GEBE access to the premises for the purpose of reading meters or examining, repairing or removing any of the Company's property; (2) notify NV GEBE at least three days in advance of the date you will no longer be responsible for electric and water service (responsibility and billing will continue until we are notified to terminate electric service). To the best of my knowledge, the information provided on this application is accurate and no attempt has been made to misrepresent the facts. I understand that providing knowingly false information on this application constitutes fraud which may result in the denial of service or disconnection.
